FOLLOW-UP VISIT (Child & Adolescent Medication Check) (30 minutes)

Follow-up medication management visits with all patients will be scheduled for thirty minutes. Medication management visits can be scheduled for 60 minutes on a case-by-case basis.

FOLLOW-UP VISIT (Adult Medication Check)
(30 minutes)

An initial visit for a Child or Adolescent patient and Family with lower to moderate complexity is completed  in about an hour. You will be asked about your medical and psychiatric history, current symptoms, and goals for treatment. At the conclusion of the visit, a treatment plan will be developed with Child, Adolescent, Parents, Family input.

EVALUATION (STANDARD ADULT- AGES 23- 65)
(60 minutes)

An initial visit for an adult patient with lower to moderate complexity is completed  in about an hour. You will be asked about your medical and psychiatric history, current symptoms, and goals for treatment. At the conclusion of the visit, a treatment plan will be developed with your input.

MEDICATION CONSULTATION
(60 minutes)

Consultation with patients who are currently treating mental health symptoms with medication and continue to struggle with symptoms. Dr. Cramer will evaluate symptoms, diagnosis, gaps in treatment ,  and treatment resistant symptoms. Dr. Cramer will  determine with you the best course of treatment.

EVALUATION (CHILD/ADOLESCENT – UNDER 18)
(90 minutes)

An initial visit for a child or adolescent patient is completed in 90 minutes. Consisting primarily of interviewing parents, and the identification of any additional information needed (lab studies, medical records); a patient interview/observation; discussion of diagnosis and treatment planning (most commonly with child and parent included).

EVALUATION (COMPLEX ADULT- AGES 18+)
(90 minutes)

For young adults (typically ages 18-22) who remain in the care of their parents or who have a significant psychiatric history, the evaluation process consists of interviewing the patient (and sometimes parent or caregiver if appropriate), a discussion of diagnosis and treatment planning. Dr. Cramer reviews relevant records and collateral information prior to the treatment planning session.

MEDICAL/PSYCHIATRIC COMORBID COMPLEXITY EVALUATION
(90 minutes)

Evaluation of Individuals with diagnosed and established treatment of medical conditions who are also seeking collaborative psychiatric services. Dr. Cramer will not offer medical diagnoses but will collaborate with your Medical Specialist and /or Primary Care Physician to support your overall medical and psychiatric care.

The initial Evaluation will focus on keeping in mind the goal of both immediate and long-term assessments so that a comprehensive but individualized , context and culturally sensitive , reality based, recovery-oriented management plan can be formulated.

A Treatment plan will be developed using history taking, Mental Status Examination, reports of established medical conditions and records. Laboratory measures may be utilized as needed. During assessment, collateral reports and past medical records are valuable additions along with patient self-report. Assessment and management will focus on the various bio-psycho-social domains relevant to the individual.
