Practice Policies

  New patients will be asked to sign and return a copy of the practice policies prior to entering treatment. The policies can be found under the FORMS section of this website.

Payment for Services Rendered

  A credit card must be kept on file (stored safely in the electronic health record system) during treatment at Elite Psychiatric Services. Payment for services is due at the time of service. Payment will be billed to the credit card held on file unless you have an accepted insurance plan, or other arrangements are made at the time of service.

Appointment Cancellation

  Keeping agreed upon regular appointments is crucial to successful treatment. We will collaborate with you to the best of our ability to find a convenient time for your appointments. The scheduling of an appointment constitutes an agreement to pay for the professional time reserved exclusively for you/your child.

Appointments cancelled with 24 hours’ notice or more will not incur a charge. Fees for late cancellations and missed appointments will be charged to the credit card on file.

Upon booking, you will receive a confirmation email with links to reschedule or cancel. If you do not make changes at least 24 hours ahead of your session, you will be subject to a no-show/late cancellation fee.

We have this policy out of respect for providers and clients. Changes less than 24 hours in advance lead to appointment slots that are difficult to fill. Without ample notice, cancellations prevent others from being able to schedule into that time slot.
