Welcome!  To schedule an initial appointment:

1. Call the Practice Office to make an appointment.  (610) 202-3440
2. The Office will assist you in creating an account on the Patient Portal.
3. A link will be sent to your email to establish your account.
4. Establish an account by creating a Username, Password and complete all Intake Forms.
5. You will then have access to the Provider calendars to self-schedule.
6. Follow-up appointments can be scheduled using your Patient Portal Account or by calling the Practice Office.

     2. Call the Practice Office to request an appointment. Phone: (610) 202-3440 
     3. Your Provider can schedule a follow-up appointment with you at the end of each appointment.
